understanding the Risks of Tattooing and Piercings *
That I have been fully informed of the inherent risks, associated with getting a tattoo. I fully understand that these risks, known and unknown, can lead to injury, including but not limited to infection, scarring, difficulties in detecting melanoma and allergic reactions to tattoo pigment, latex gloves, and/or soap. Having been informed of the potential risks, I still wish to proceed with the tattoo application and I freely accept and expressly assume any and all risks.
Waiving your right*
TO WAIVE AND RELEASE to the fullest extent permitted by law each of the Artist and the Studio from all liability whatsoever, for any and all claims or causes of action that I, my estate, heirs, executors or assigns may have for personal injury or otherwise, including any direct and/or consequential damages, which result or arise from my tattoo, whether caused by the negligence or fault of either the Artist or the Tattoo Studio, or otherwise.
Healing & Aftercare Requirements to follow for Free Touch Ups *
We Email you a Full detailed copy of your aftercare sheet along with video, and audio files to cover all our bases. The Artists and the Tattoo & Piercing Studio have given me verbal instructions on the care of my tattoo/piercing after it was done and instructed me on what to do while it's healing, and I understand them and will follow the provided aftercare giving. I acknowledge that it is possible that the tattoo can become infected, particularly if I do NOT follow the instructions given to me. To qualify for FREE touch-up work if needed due to my own negligence, I agree that the work will be done at my own expense and not the artists/studios time withing the first 90 days of the Tattoo.

If you Do any of the following during your Healing time of the tattoo this will VOID your free touch up and you will be charged hourly with us.
What Not to do so you Qualify for free touch ups:
-Do not cover your tattoo with sunblock until it’s fully healed
-Do not scratch or pick at the tattoo.
-Do not wear tight clothing over the tattoo
-Do not go swimming or immerse your body in fresh water/saltwater (showers & shorth baths are fine)
-Do not go to the gym or workout.
-Do not play any sports or do any activity's that causes heavy sweating.
-Do not touch your tattoo without washing your hands prior.
-Do not take the tagedurm/sanadurm bandage off before recommended time giving in the Aftercare text/video/audio instructions emailed to you after you complete this form.
-No excessive sun exposure or tanning
-Do not over coat your tattoo with ANY product, This may cause the pours of your skin to close and you can trap bacteria on the surface of your skin and cause infection.

I do not have diabetes, epilepsy, hemophilia, a heart condition, nor do I take blood thinning medication. I do not have any other condition that may interfere with the application or healing of the tattoo. I am not the recipient of an organ or bone marrow transplant or, if I am, I have taken the preventive anti-biotics. I am not pregnant or nursing. I do not have a mental impairment that may affect my judgment in getting the tattoo.

Spelling is your responsibility *
Neither the Artist nor the Tattoo Studio is responsible for the meaning or spelling of the symbol or text that have provided to them or chosen by the artists or flash (design) sheets that were provided or recommended prior to your appointment.

-MAKE SURE YOU SPELL CHECK BEFORE THE TATTOO!
Fading over time*
Variations in color/design may exist between the art I have selected or allowed and the actual tattoo may vary. I understand that over time, the colors and the clarity of my tattoo will fade due to natural dispersion of pigment under the skin as age accrues.

-Fading does not Qualify for FREE Touch ups over 90 days.
Permanent change to the body & TOUCH UPS *
A tattoo is a permanent change to my appearance and can only be removed by laser or surgical means, which can be disfiguring and/or costly and which in all likelihood will not result in the restoration of my skin. Reef side Tattoo offers their clients FREE touch ups on all tattoos as long as the client follows the correct Aftercare instructions and does not have any sighs of improper healing technics with in the first 90 days after the First Tattoo Appointment with Reef Side Tattoo. However we do offer touch ups on tattoos done by the same artist for tattoos if the tattoo session is under 30 mins of tattoo time and its the same artist.
Legal Action*
I agree to reimburse each of the Artist and the Tattoo Studio for any attorneys' fees and costs incurred in any legal action I bring against either the Artist or the Tattoo Studio and in which either the Artist or the Tattoo Studio is the prevailing party. I agree that the that the courts of [FLORIDA] in [USA] shall have personal jurisdiction and venue over me and shall have exclusive jurisdiction for the purpose of litigating any dispute arising out of or related to this agreement. I shall not Slander or use defamation of Reef Side Tattoos name via online or verbally and shall bot verbally attack the reputation via online or verbally no matter the out come of the body modification.I understand the risks and release full liability of the business LLC Reef Side Tattoo. And understand all Sub contractors hired by Reef Side Tattoo are 1099 employees and shall not Slander the business in anyway.
Questions or changes*
I acknowledge that I have been given adequate opportunity to read and understand this document, that any and all of my questions have been answered, that it was not presented to me at the last minute, and I understand that I am signing a legal contract waiving certain rights to recover against the Artist and the Tattoo Studio.
Photography release*
I release all rights to any photographs taken of me and the tattoo and give consent in advance to their reproduction in print or electronic form. (If you do not tick this provision, please advise your Artist).
