I acknowledge that by signing this agreement I have been given the opportunity to ask any and all questions which I have about the obtaining of a tattoo and that all of my questions have been answered to my full satisfaction. I specifically acknowledge I:

I consent to the final design of my tattoo, and will express any feedback or changes before the tattooing process begins.

I am not pregnant or nursing, under the influence of alcohol or drugs, or taking medication that thins the blood.

I have advised my tattooer if I have any communicable disease or heart condition such as diabetes, epilepsy, hepatitis, hemophilia, HIV-AIDS.

I do not have medical or skin conditions that may interfere with said tattoo such as but not limited to: acne, scarring (Keloid), eczema, freckles, moles or sunburn.

I acknowledge it’s not reasonable for the tattooer or employees of Nice Tattoo to determine whether I might have an allergic reaction to the pigments or processes in my tattoo, and as such I agree to accept the risk that such a reaction is possible.

I acknowledge that infection is possible as a result of obtaining my tattoo, particularly in the event that I do not take proper care of my tattoo, and assume this risk. I have received aftercare instructions and I agree to follow them while my tattoo is healing.

I agree that any follow-up work on my tattoo, due to my own negligence, will be done at my own expense, especially misspellings or designs that fill me with regret later.

I understand that size, line thickness, color, placement, and overall design are factors in the longevity of my tattoo (e.g., ultra fine-line, finger, and foot tattoos may fade)

I realize that variations in color and design exist between any tattoo design and any tattoo applied to a body, due to the fact that ink is being deposited under a layer of skin.

I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo.

I acknowledge that the tattoo artist is not a medical professional, and any suggestions are not construed as or substituted for advice from any medical professional.

I acknowledge that while being tattooed, employees of Nice Tattoo may photograph or film the process. I release this content for commercial and non-commercial usage.

I acknowledge I am over the age of eighteen and I have truthfully represented this to my tattooer, and that obtaining my tattoo is my choice alone. I consent to the application of the tattoo and to any actions or conduct of the representatives and employees of Nice Tattoo reasonably necessary to perform the tattoo procedure.

I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo. In addition, to my knowledge I don’t have a physical, mental or medical impairment or disability, which might affect my well being as a result of my tattoo.
If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
