Acknowledgement of missing vaccination records

I understand that proof of vaccination for rabies, DHPP and bordetella for dogs and proof of rabies for cats is required by law to receive any service at Animal Loving Care (unless I have proof of medical exemption from any of these vaccines). I understand that despite ALC’s best efforts to remind me to obtain proper proof of vaccinations before my arrival, I have been unable to do so or the records I have brought do not reflect vaccine(s) in the proper format.

I understand that by choosing to leave my pet with Animal Loving Care today I am acknowledging that I will provide the appropriate records in as timely a fashion as possible. I also understand that in the unlikely event that the NYC Dept. of Health and Mental Hygiene come for a random inspection and ask for proof of vaccinations for my pet I will be held solely financially responsible for any penalties imposed on Animal Loving Care ($200 per missing vaccine).
If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
