The NY State Health Department requires bordetella (kennel cough vaccine) be current for dogs for all services in which they will be in the presence of other dogs including grooming, daycare, and boarding. Any dog entering our facility must be vaccinated for kennel cough with paperwork stating the expiration date of this vaccine in addition to rabies and distemper (dhpp). Bordetella  can be an effective and helpful barrier to kennel cough. While it does not offer 100% protection against the disease, vaccines are one of several steps owners can take to prevent severe kennel cough infections in their puppy or dog. As a courtesy to our customers, we are now providing this vaccine via oral administration.

I, ____________________________ , have opted to have bordetella given to my pet at Animal Loving Care. I understand that it could take up to 14 days after the date of administration for the vaccine to be fully effective. Possible side effects include temporary lack of appetite, lethargy and low fever. These side effects are a natural response to the vaccine\'s effect on the immune system. If your dog has these symptoms, keep watch over him and make sure he is drinking water. Some dogs have allergic reactions or drug interactions with vaccines, so always talk to your vet about any previous reactions to medications or vaccinations. Animal Loving Care will not be held liable in any way for failure to follow the recommended guidelines or for any potential side effects. I understand that I will be responsible for covering the costs of bringing my dog to a veterinarian should there be any complications from the vaccine.

If any provision, section, subsection, clause or phrase of this release is found to be unenforceable or invalid, that portion shall be severed from this contract. The remainder of this contract will then be construed as though the unenforceable portion had never been contained in this document.
